Adopted Forms for SR Providers
The forms below comprise the statewide provider contract for the School Readiness program. Providers must complete the forms in the Provider Portal. All providers must complete Form DEL-SR 20 and either Form DEL-SR-20L (for licensed providers), 20LE (for license-exempt providers), or 20FFN (for informatl providers). Only after a Statewide SR Provider Contract has been executed by both the provider and an early learning coalition may a provider complete Form DEL-SR 20A to amend the Statewide Provider Contract as described within the form.
